Flourish Neuropsychology is currently not in-network with any insurance providers. As an out-of-network provider, we are happy to provide you with the necessary documentation (called a “superbill”) that you can submit directly to your insurance provider for potential reimbursement. Reimbursement is not guaranteed, so we recommend contacting your insurance provider to find out the details of your plan.
If you would like to pursue potential insurance reimbursement, we recommend contacting your insurance provider to verify whether your plan offers out-of-network benefits for telehealth mental health services. If these benefits are available, we recommend that you ask your insurance provider about the following:
How do I submit my superbill documentation for reimbursement? (Ask for the claims address if needed to submit.)
How much does my plan reimburse for out-of-network telehealth mental health services?
Ask about the following CPT codes: 90791, 96116, 96130, 96131, 96132, 96133, 96136, 96137
Do I need prior authorization or approval from my primary care physician?
If so, what do I need to do to obtain authorization?
If I have a prior authorization required, what is the effective date and expiration date?
What is my annual deductible?
Have I met the deductible for this year already?
If your insurance says that you do not have out-of-network benefits, you can ask for an in-network provider that is available with the appropriate specialization to serve your needs in a more timely manner (i.e., not a 10-12 month wait on a waitlist).
Waitlists can be very long for some in-network providers. To see a provider with the specialty you need, you can request an appeal or exception, with your insurance to advocate for out-of-network care options since insurance needs to provide you with appropriate care.
I can help you complete paperwork to file an appeal or exception with insurance if you do not have out-of-network benefits. If your appeal is approved, my services could potentially be reimbursed to you by insurance as if I were an in-network provider.

Our screenings are open to all ages (PreK through Adult). For example, $3,500 (comprehensive neuropsychological evaluation) - $750 (screening evaluation) = $2,750.
